4. Click on the link for payment and ensure the appointment is confirmed. You may need to deposit a specified amount to book the appointment.
5. Take the printout of the application receipt, and on the scheduled date, visit the nearest Passport Office or Passport Seva Kendra for document verification.

Documents Required for Name Change:

• Marriage certificate (original and photocopy)
• Spouse’s passport’s self-attested photocopy
• Proof of current address (Aadhar card, electricity bill, water bill, etc.)
• Self-attested photocopy of pages in the old passport with the changed name, if any
• Verification pages and additional booklet pages, if applicable
